As stated in the VCHA Medical/Dental Staff Rules, Active and Provisional Staff are required to pay annual staff dues at their primary site only. Dues are $100 for all regular staff and $50 for salaried staff who are unable to deduct their dues.The deadline is March 29, 2019. Applications received without payment will be considered incomplete and your reappointment will not be processed.
There are three payment options:
- By Cheque: Please make cheque payable to: VMDAS Association and staple to this form and forward to Frances Perry in the Vancouver Acute Department of Medical Affairs.
- By Credit Card: Pay online at https://ourvancouvermsa.ca/product-category/dues/
- By Cash: Payments are accepted Monday to Friday, 0900–1530 hours. Please contact Frances Perry at (604) 875-4111 ext. 66271 to arrange.
If you are NOT paying at Vancouver Acute (VGH/UBCH/GF Strong), please fill out the bottom portion of this form and return along with your Application for Reappointment documents OR email Frances.Perry@vch.ca to notify her at which site you are paying your dues.
A receipt for payment will be mailed to you with your Reappointment letter in August 2019.
