As stated in the VCHA Medical/Dental Staff Rules, Active and Provisional Staff shall be required to pay annual staff dues at their primary site only. Dues are $100 for all regular staff and $50 for salaried staff who are unable to deduct their dues. The deadline is March 29, 2018. Applications received without payment will be considered incomplete and your reappointment will not be processed.
There are three payment options:
- By cheque: Please make cheque payable to VHHSC Medical and Allied Staff Fund and staple to this form and forward to Jennifer Laxamana in the Vancouver Acute Dept. of Medical Affairs.
- By credit card: Please pay via the online cart and choose either:
- $100 or
- $50 (For salaried Vancouver CoC Medical / Dental Staff members who are unable to deduct their dues, the 2018 dues are $50)
- By cash: Payments are accepted Monday to Friday, 0900–1530 hrs. Please contact Jennifer Laxamana at (604)875-4111 ext 66271 to arrange.
If you are NOT paying at Vancouver Acute (VGH/UBCH/GF Strong), please fill out the bottom portion of this form and return along with your Application for Reappointment documents OR email Jennifer.Laxamana@vch.ca to notify her which site you are paying your dues.
A receipt for payment will be mailed to you with your Reappointment letter in August 2018.
